| Description |
| Member goes to elderly aunt's house to help her with hygiene cooking and cleaning 2x week. Following suggestions by Home Nurse PT and Doctor the goal is for member's aunt to become self sufficient that includes being alone at times going out more with friends going into public places etc. Eventually aunt will relocate into an assisted living location. Number of visits have dropped and she is receiving meals by delivery and makes her own sandwiches or heats food up in the microwave. She is improving after her stroke and surgery. |
